Instructions for Travel Visa Application:
A) Provide your PHYSICAL passport.
- The Passport MUST be signed.
- Your Passport MUST have (2) blank visa pages that are adjacent to each other.
- The Passport Must have at least (6) months remaining validity from date of entry.
Note: Please be advised that your passport should be undamaged. Passports should be free of fraying, tears, damage to the credential page and pages that have been separated from the binding.
B) Complete (2) Online Indonesia Visa Application Forms. NOTE: Effective September 2015 the Consulate of Indonesia in Houston will only accept forms completed online. Typed or hand-written forms will not be accepted.
C) Two (2) Passport-Size Photographs. (Click to review specific requirements. Incorrect photographs may cause a delay or rejection of visa issuance).
D) Bank Statement. (A copy of the applicant’s most recent Bank Statement showing evidence of sufficient funds for travel to Indonesia, a minimum balance of $2,000.00USD is required).
E) Proof of Travel. (A copy of applicant’s itinerary, airline ticket or e-mail confirmation).
F) Proof of Residency. (Copy of applicant's valid Driver's License).
G) Business Letter of Financial Guarantee:Provide an original Business Letter from your current employer or sponsoring company in the United States (fax and/or scan copy NOT acceptable).
H) Business Letter of Invitation. Provide a Letter of Invitation from the company in Indonesia that you will be visiting (clear fax and/or scan copy acceptable). Required only for Multiple Entry requests.
I) Complete TPAVC Order Form and payment.
*NOTE: U.S. Passport holders are eligible to obtain a Visa on Arrival when you are landed at certain airports and seaports in Indonesia regardless of the purpose of your visit. The maximum stay permitted for the Visa on Arrival is 30 days ($30.00USD)
